The TRAC intake form should be used to submit requests for instructional teaching resources. Please gather all requested information before starting the form.
Please submit one request per area of instructional need, for which you can include up to five faculty appointment requests. Academic units requesting more than five faculty appointments will be asked to complete the TRAC intake form via an Excel workbook. The information requested in the workbook is the same as the information requested in the online form.
After submitting the form, you will receive an email confirmation that will include your responses.
General Information:

- Appointment type (select one of: instructional faculty member, emeritus faculty member, visiting ladder faculty member, staff with teaching appointment, postdoc/researcher with teaching appointment, professional school PTAI)

2The deadline to submit requests for searches and multi-year conversion requests is 9/25/2026. After this date, these options will no longer be available.
