Requested information for TRAC intake form

List of requested information on the TRAC intake form, used by academic units to request instructional faculty resources.

The TRAC intake form should be used to submit requests for instructional teaching resources. Gather all requested information prior to starting the form. Submit separate forms for each request.

  • Email address (text entry)
  • Academic unit (dropdown)
  • Area of instructional need (text entry)
  • Is this a request to fill a vacated position? (yes/no radio buttons)
    • [if yes selected] Please provide the name of the person who last filled the position. (text entry)
  • What is the anticipated start date of the requested appointment? (date picker)
  • Will the requested appointment be for a single semester, single year, or multiyear? (radio buttons)
  • What type of appointment are you requesting? (radio buttons)
    • Select one option: instructional faculty member, emeritus faculty member, visiting faculty member, postdoc with teaching appointment
  • How many faculty in [area of instructional need] are you requesting to appoint? (text entry)
    • This will by default list "1". If multiple appointments are needed for the area of instructional need, update the answer to the appropriate number.
  • Will the requested appointment be full-time or part-time? (radio buttons)
    • What are the expected academic responsibilities (select all that apply)? (radio buttons)
      • Visible options depend on selecting part-time or full-time and whether single semester or single year or multiyear are selected: 1 term course, 2 term courses, 3 term courses, 4 term courses, 5 term courses, 6 term courses, 1 course equivalent of service, 2 course equivalents of service
    • Do you already have a candidate you would like to hire? (yes/no radio buttons)
      • [if yes selected], Candidate's name and current academic affiliation. (text entry)
        • (optional) If available for the selected candidate, provide as a single PDF: CV, cover letter, 2 letters of reference, and any other relevant documents. (file upload)
      • [if no selected], Would you like to conduct a full search with posted advertisement and a search committee? (yes/no radio buttons)
  • Would the candidate teach any course that is either new to the curriculum or was last offered more than 5 years ago? (yes/no radio buttons)
  • For each term course that would be taught by the candidate, please provide 1) course number, 2) title, 3) format (seminar, lecture, or lab), and 4) most recent enrollment. (text entry)
  • For each term course that would be taught by the candidate, please describe how it fits within the curriculum (e.g., fulfilling degree program requirements, supporting certificates or specializations within a major, meeting distribution requirements, providing elective options, etc.) and what the impact would be of not offering the course next year. (text entry)
  • For each term course that would be taught by the candidate, please explain why it cannot be taught by an existing instructor (ladder faculty, instructional faculty, postdoctoral appointees, graduate students). (text entry)
  • (optional) Please share any other relevant information in support of this request (~300 word maximum). (text entry)

There is an additional question if the academic unit is East Asian Languages and Literatures; English; French; Germanic Languages and Literatures; Italian Studies; Linguistics; MacMillan Center; Near Eastern Languages and Civilizations; Slavic Languages and Literatures; Theater, Dance, and Performance Studies: Does this request fall within this program's section allocation? (yes/no/not applicable radio buttons). If yes is selected, the course-related questions are skipped.
